matching curriculum_topic_id).
Generate questions
One run = one LLM call = up to 25 questions for a single (country, subject, topic, difficulty). The LLM sometimes returns fewer items than requested — if so, re-run the same topic to top up. Duplicates of existing questions are skipped automatically and reported in the result.
